WASHINGTON −The Bureau of Safety and Environmental Enforcement today released the report of its Panel Investigation into a Dec. 2, 2017 fatality aboard the Petrobras 10000 drillship working in the Walker Ridge 469 area of the Gulf of Mexico, about 240 miles southwest of New Orleans. The panel, including BSEE subject matter experts, engineers, and investigators, conducted its investigation of the incident to determine the cause and contributing factors that led to the fatality of a floorhand employed by Transocean Offshore Deepwater Drilling, Inc.
The floorhand was performing drill pipe picking up and racking back operations on the auxiliary drill floor when he was fatally injured.
After a thorough investigation and analysis of the incident, BSEE’s panel made several recommendations to reduce the likelihood of similar events in the future.
