Provides guidance and instructions on disposal of E&P waste, worker safety when handling waste, and contents of applications to dispose of or store E&P wastes.   Includes worker safety guidelines for handling NORM waste when above background levels, and requires written disclosure of the lease number, area, block, and well number of any disposal wells.
